Registration Fee Includes
Educational workshop on Friday-Sunday, handout materials, coffee breaks, lunches, 1 ticket to Welcome Reception with Dinner on Friday, 1 ticket to Gala Dinner on Saturday.

Non-Attendee Food & Beverage Fee (Spouse/Guest)
Spouse/guest(s) may join the two evening functions (Welcome Reception with Dinner on Friday and Gala Dinner on Saturday evening) for additional fee of $95.  You may indicate this during the registration process. 

Cancellation/Refund Policy
Registration fees, less a $100 administration fee, will be refunded upon written notice of cancellation to the Workshop Registrar received on or before February 1st , 2019. There will be no refund of fees for cancellation or for lack of attendance without notification. It is your responsibility to ensure that your cancellation request has been received by the Workshop Registrar. "No shows" that have not pre-paid will be invoiced for the total registration fee.
